Medical Record Services
At PMC, our Medical Records Department plays a vital role in supporting patient care by ensuring a secure, accurate, and timely management of patient documentation. Our goal is to provide quality and timely services to all stakeholders by ensuring the availability of patient-related documentation to facilitate optimum patient care and services.
How to Request Medical Records
Patients or authorized representatives may request access to medical records by contacting our department via email at medicalreportpmc@asia1health.com or by visiting our Medical Records department in person. To initiate a request, requestors are required to complete the Consent for Release of Medical Information form signed by patient, which can be accessible here.
All requests must be accompanied by a valid patient identification card or passport and authorization documents. Please ensure that all required forms and documents are submitted to avoid any delays. Once the documents are submitted, our team will verify the request and begin processing.
Processing Time
The minimum processing period is 14 to 28 working days, depending on the nature and complexity of the request. Under certain circumstances, processing time may be extended.
Operating Hours
Monday – Friday: 8:30am – 5:00pm
Saturday: 8:30am – 12:30pm
Closed on Sunday & Public Holidays.
